[Abstract] Objective  To explore the evaluation effect of high-field magnetic resonance iterative decomposition of water and fat with echo asymmetry and least squares estimation quantification sequence (IDEAL-IQ) in fat infiltration in acute non-biliary pancreatitis in order to provide value of clinical application for the diagnosis of acute non-biliary pancreatitis. Methods A retrospective analysis was conducted on 35 patients with acute non-biliary pancreatitis who were treated in our hospital from October 2016 to October 2019 and 35 normal controls who were recruited during the same period. The cubital venous blood was drawn for the detection of serum Amy, Lps, TC, and TG. APACHE II scores were classified. Quantitative analyses of MRI T1W1, T2W1 and IDEAL-IQ were conducted. The fraction value (FF value), R2* value, and correlation with laboratory examination were compared between the two groups. Results The serum Amy, Lps, TC, and TG and APACHE II score of the control group were lower than those of the observation group, with statistically significant differences (P<0.05). The FF values and R2* values of each location of pancreas and whole pancreas of the control group were lower than those of the observation group, with statistically significant differences (P<0.05). The FF values and R2* values of the two groups were moderately positively correlated with serum Amy, Lps, TC, and TG, and were weakly correlated with APACHEⅡ. Conclusion The high-field magnetic resonance water and fat model quantitative analysis quantitatively analyzes the pancreatic fat infiltration in patients with acute non-biliary pancreatitis. It is non-invasive and safe, with good repeatability and reliable results, showing important guiding value for clinical diagnosis.
