人工流产同时上环联合人工周期治疗对预防术后宫腔粘连的效果探讨

[摘要] 目的 探讨分析人工流产同时上环联合人工周期治疗对预防术后宫腔粘连的效果。方法 方便选取该院2018年10月—2019年10月收治的98例行人工流产的患者，随机分为对照组和研究组，给予对照组患者人工流产术联合妈富隆治疗，给予研究组患者人工流产同时上环联合人工周期治疗，比较分析两组患者治疗效果。结果 在给予两组患者不同治疗措施后，对照组患者宫腔粘连率为32.65%，研究组患者宫腔粘连率为4.08%，两组患者在术后宫腔粘连指标上差异有统计学意义（χ2=13.339，P<0.05）;研究组患者在月经恢复时间（28.32±3.77）d、术后阴道出血时间（4.28±2.39）d上均显著优于对照组[（32.24±4.10）、（5.98±2.64）d]，差異有统计学意义（P<0.05）;研究组患者月经量正常率为95.92%，对照组患者月经量正常率为26.53%，两组患者在月经量上差异有统计学意义（χ2=49.688，P<0.05）。结论 人工流产同时上环联合人工周期治疗方式可以有效预防宫腔粘连，提升避孕效果，还有助于改善术后月经周期紊乱症状，尽早恢复正常月经。

The Effect of Artificial Abortion and Upper Ring Combined with Artificial Cycle Treatment on Preventing Postoperative Intrauterine Adhesions

[Abstract] Objective To analyze the effect of artificial abortion and upper ring combined with artificial cycle treatment on preventing postoperative intrauterine adhesions. Methods A total of 98 patients undergoing artificial abortion admitted to the hospital from October 2018 to October 2019 were selected and randomly divided into control group and study group. Patients in the control group were given artificial abortion combined with Marvelon treatment， and patients in the study group were given artificial abortion at the same time， the upper ring combined with artificial cycle treatment， the treatment effect of the two groups of patients was compared and analyzed. Results After different treatment measures were given to the two groups of patients， the intrauterine adhesion rate of the control group was 32.65%， and the study group was 4.08%. There were statistically significant differences in the index of postoperative intrauterine adhesion between the two groups （χ2=13.339，P<0.05）; the patients in the study group in menstrual recovery time （28.32±3.77）d and postoperative vaginal bleeding time （4.28±2.39）d were significantly better than the control group （32.24±4.10）d and （5.98±2.64）d. the difference was statistically significant（P<0.05）. The normal menstrual volume rate of the study group was 95.92%， and the control group had a normal menstrual volume rate of 26.53%， the difference was statistically significant （χ2=49.688，P<0.05）. Conclusion The treatment of artificial abortion and upper ring combined with artificial cycle can effectively prevent intrauterine adhesions， improve the contraceptive effect， and also help to improve the symptoms of postoperative menstrual cycle disorders and restore normal menstruation as soon as possible.

在采用人工流产术即刻放置节育环后，及时给予患者克龄蒙药物，可以促进子宫内膜增生与修复，还能够维护子宫内膜完整性，尽早恢复月经周期[9-10]。同时，克龄蒙药物还能够降低宫腔粘连率，增加宫腔粘液的粘稠性，以免细菌从阴道内进入到宫腔，还可以降低术后盆腔感染率。通过此次研究能够看出，对照组患者宫腔粘连率为32.65%，研究组患者宫腔粘连率为4.08%，两组患者比较存在显著差异，验证了人工流产术即刻放置节育环后服用克龄蒙药物对宫腔粘连症状的预防效果。研究组患者在月经恢复时间（28.32±3.77）d、术后阴道出血时间（4.28±2.39）d上均显著优于对照组（32.24±4.10）d、（5.98±2.64）d（P<0.05）。此次研究也与相关学者学者[11]的研究结果相同，在该学者的研究报道中，给予患者不同治疗措施后，观察组月经恢复时间为（29.2±3.6）d，阴道出血时间为（4.1±2.2）d;对比1组患者的月经恢复时间为（30.9±3.7）d，阴道出血时间为（5.6±2.3）d。通过此次对接受人工流产术患者的治疗与护理，有助于了解术后宫腔粘连的治疗要点，希望能够为临床治疗与护理提供一定的价值。

综上所述，人工流产同时上环联合人工周期治疗方式可以有效预防宫腔粘连，提升避孕效果，还有助于改善术后月经周期紊乱症状，尽早恢复正常月经。
